ecpg shouldn't depend on parser/gramparse.h. Also, eliminate some

compiler warnings caused by lack of extern declarations in extern.h.
I believe the remaining gcc warnings here would go away if the ecpg
grammar could be tweaked so it doesn't use REJECT ...
